The IU Health Center destroys medical records that have remained inactive for a period of seven years. Those who wish to have their personal records or to have them forwarded to a physician or a health-care facility when the medical record has met the retention deadline, may submit requests to:
Medical Record Administrator, IU Health Center, 600 N. Jordan Avenue, Bloomington, IN 47405.
Requests should include name, both signed and printed, and those who had a different name while in school should include that information; Social Security number and/or student identification number; and the complete name and address where records should be sent.
